APPOINTMENT REMINDERS
Please allow plenty of time to
park and arrive at our office on time. We recommend arriving 20 minutes before your appointment time in order to confirm
your insurance carrier prior to seeing one of our pacticioners.
If you need to cancel or reschedule a medical, or surgical appointment, please
provide one working day's notice. We will be happy to reschedule your appointment.
We try to
honor your appointment time, as we firmly believe in the value of your time. Please remember that in our specialty there
may be unforseen delays that may impact our schedules; for example, babies being born, surgical delays, emergency patients,
etc. If timing is critical to your schedule on a particular day, notify the receptionist so that we can make every effort
possible to accommodate your needs.
PRESCRIPTION
REFILLS
In most cases you have been given several refills when the prescription was
writen. Please request all refill prescriptions through your pharmacist. If necessary, the pharmacist will call your
physician to obtain approval for a refill. Some medications will require periodic checkups to maintain usage; for these medications,
you must make an appointment with your physician to receive a new prescription
